- To print an application from the portal, the URL is https://www.thecardservicescenter.com/
- You will utilize your CUE credentials to log in. If you have forgotten your password, you will need to reset your CUE password first before accessing the resource portal.
- Once in the portal, Click the Documents tab on the left and the Applications tab at the top.
- Choose the type of application you will need.
- On the right side, choose Print Application and from there you can print or download the application.
- Do not forget to write in your bank's agent ID number on each application on the top right, and if you participate in the AMPD Rewards program, fill out the FI Use only section.
